Project Homeless Connect 2026: Health & Wellness Clinic Service Providers

Thank you for providing health & wellness services for Project Homeless Connect, to be held on Friday, January 23, 2026! When you "respond" to this need, you will be asked a series of questions to help ensure you have what you need to provide services at the event.  We also ask that you print the waiver that becomes available when you "respond" to this need. (To access the waiver, click the link labeled "I have read and agree to the volunteer waiver" next to the blue SUBMIT NEED RESPONSE button. Everyone on your team who participates in Project Homeless Connect will need to sign a copy of this waiver and bring it with them to the event. When they check-in at the event, they will be asked for a signed copy of the waiver. Please note that if your services do not relate to the Health & Wellness Clinic component of the event, there is a separate link you should use. These components are being managed by different team leads.
